New Patient Registration

If you live within our practice area you are welcome to register with us and our reception staff will be happy to guide you through the procedure. Please phone reception, or print off the registration form here and drop it off at the surgery. 

As a guide the practice covers Alum Rock, Washwood Heath, Saltley, Ward End, Hodge Hill, Bordesley Green, Small Heath and Nechells. (Birmingham B7/ B8/ B9/ B10, part of Hodge Hill B36 and part of Stechford B34).

You will need to complete a registration form (GMS1) and a health questionnaire which will provide useful information whilst we wait for your medical records to arrive from your previous doctor.

Please bring an accurate note of any repeat medication you may be taking.

Changes to Personal Details

If you change your home or mobile number, it is very important to inform the practice so that we can keep our records up-to-date. If you move outside the practice area, please check with the reception to see if we can still continue to care for you.
